Evidence:
1. In the Jr. Toddler Classroom, the emergency contact list for children was posted on the board near the teacher desk that included names, phone numbers and relationship to the child.

Plan of Correction: Staff have been retrained and the record has been removed.

Description: Based on review of the orientation documentation, the center did not document all required information.
Evidence:
1. Staff #4's orientation documentation was missing the delivery method and the entity or individual providing training.

Description: Based on observation, equipment of the center inside were not maintained in a safe and operable condition.
Evidence:
1. In the Jr. Toddler Classroom, one yellow wood chair had a large chip in the wood on the back of the chair and another yellow wood chair back was coming apart (the peg was out of the slot).

Plan of Correction: Replace the chairs

Based on observation, hazardous substances were not kept in a locked place using a safe
locking method that prevents access by children.
Evidence:
1. In the Sr. Toddler classroom, a spray bottle of bleach water was on the wall basket and not kept in a locked place.
2. In the Hall bathroom, a spray bottle of Febreze was next to the sink. The door to the bathroom was open.

Plan of Correction: 1. Retrain the staff to lock the bleach
bottle in the cabinet
2. Keep the adult bathroom door locked
when not in use.

Description: Repeat Violation
Based on observation, the center did not meet all diaper ointment or cream requirements.
Evidence:
1. In the infant classroom, two diaper creams were in low cubbies and were within reach of children and accessible.

Plan of Correction: The diaper creams are now put away in the cabinet.

Standard #: 8VAC20-780-550-E
Description: Based on review of drill records, the center did not practice shelter in place procedures a minimum of twice per year.
Evidence:
1. The center documented one shelter in place drill on 4/27/2023, but did not document a 2nd drill in 2023.

Plan of Correction: All drilled are now scheduled in the calendar with reminders.
