Comments:
A monitoring inspection was conducted today between 9:00am and 10:30am. There were 70 children present with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing outside, practicing math skills, building, and center time. A total of 7 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued. Standard #: 8VAC20-780-70
Description: Based on review of 5 staff records, not all records were complete.
Evidence
The record for staff #1 contained documentation of only 1 or the required 2 reference checks.
The record for staff #2 did not contain documentation of 2 required reference checks.

Plan of Correction: Going forward I will make sure two references are always checked for potential staff.

Standard #: 8VAC20-780-240-E
Description: Based on review of staff records, the center did not ensure within 30 days of the first day of employment that staff completed orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence
Staff #3, first day of employment 1/9/24, does not have documentation of completing orientation training in first aid and cardiopulmonary resuscitation (CPR).

Plan of Correction: Staff is enrolled in the upcoming CPR class.

Standard #: 8VAC20-780-420-E-3
Description: Based on review of children's records, the center did not request at least annually parent confirmation that the required information in the child's record is up to date.
Evidence
The record for child #1 did not have documentation at least annually of parental confirmation that the required information in the child's records is up to date. Last annual update was 10/12/2022.

Plan of Correction: We will make sure all children's records are reviewed with the parent annually.

Standard #: 8VAC20-780-520-C
Description: Based on observation, diaper ointment was not inaccessible to children.
Evidence
A tub of diaper ointment was located on the floor of the Pre K bathroom, making them easily accessible to the children.

Plan of Correction: Corrected during inspection. I will speak with staff to ensure topicals are always inaccessible to the children.

Standard #: 8VAC20-780-550-P
Description: Based on review of written record of children's serious and minor injuries, not all records contained the required documentation.
Evidence
Not all written records of children's serious and minor injuries contained when and how the parent was notified, or a plan to prevent the accident from occurring in the future.

Plan of Correction: We will begin immediately reviewing the reports before they are signed to ensure all required information is included.
