Violations:
Standard #: 8VAC20-780-340-F
Description: Based on a review of documentation and interviews, the center did not ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.

Evidence:
Child #1 and child #2 were in a back office without supervision for approximately 8 minutes per staff interviews and the parent of child #1. Staff were not aware that two children were missing until they were located in the back office.
On March 11, 2024, at approximately 4:30 PM, staff #1 and staff #2 transitioned the toddler class from the playground back to their classroom. It was reported that staff did not complete name to face check when entering the toddler classroom. The parent of child #1 (22-month-old) entered the classroom to find that the child was not in the classroom. Staff #1, staff #2 and the parent of child #1 began searching the building for child #1. When staff were unable to locate child #1, they notified an administrator and other staff. At approximately 4:40 PM, child #1 and child #2 (18-month-old) were found in a back office inside of the preschool. When child #1 and #2 were found, both were unharmed and playing with materials in the room.
