Comments:
An unannounced renewal inspection was conducted on 5/06/2024 from 9:13am to 1:00pm. At he time of entrance, 104 children were in care with 21 staff members present. Children were observed in circle time for their morning meeting discussing celebrations, exercising in the fitness room jumping in hoops on the floor, playing in centers, playing with sensory toys, eating snack, washing hands, playing outside, singing songs, reading books, making a tissue art project inspired by an artist they learned about in class. 9 staff records and 9 children's records were reviewed. The physical space, evacuation drills, medication, first aid kit, radio, flashlight, the injury prevention plan and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Walls and bulletin boards were decorated with the children's artwork. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.


Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-780-160-A
Description: Repeat Violation
Based on review of staff records, the center did not have documentation of a
tuberculosis (TB) test or screening for two staff at the time of employment and prior to coming into contact with children that was completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #7 (Date of hire 3/19/2024) had a TB test or screening dated 3/20/2024.
2. Staff #9 (Date of hire 11/01/2022) had a TB test or screening dated 11/02/2022).

Plan of Correction: We will ensure that TB results will be received on their first day of employment.

Standard #: 8VAC20-780-160-C
Description: Based on review of staff records, the center did not obtain updated tuberculosis tests or screenings for each staff member every two years from the date of the first initial screening or testing.
Evidence:
1. Staff #1 had a tb test or screening dated 4/11/2022 and did not have an updated test or screening in the staff file.

Plan of Correction: The staff member stated her appointment is for tomorrow. We will ensure that staff members obtain their TB renewals on time.

Description: Repeat Violation
Based on observation, areas of the center inside were not maintained in a safe and
operable condition.
Evidence:
1. In the Koalas classroom, the paint or drywall was chipping near the sink approximately 1 inch in length.

Plan of Correction: We will submit a maintenance request today for the repair.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Whales classroom, a multipurpose disinfecting solution was on the top shelf of an unlocked cabinet.

Plan of Correction: Staff will be reminded to lock up these products and expired products will be disposed of.
