A monitoring inspection was initiated on 04/30/2024 and concluded on 04/30/2024 from 12:05 PM to 2:10 PM. There were 71 children present, ranging in ages from infants to five, with 14 staff and two administrators. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of seven children?s records and the three staff?s records were reviewed/updated.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-780-500-B Description: Based on observation, the center failed to ensure that a nonabsorbent surface for diapering was used or that that the diapering surface shall only be used for diapering or cleaning children.
Evidence:
1. The diaper changing table pad, located in the infant 2 classroom, had two holes exposing the foam making it absorbent.
2. Staff verified that the diaper changing pad had two holes and the foam was exposed.
3. In the three year old classroom, there were items for the new bulletin board sitting on the diapering changing table.
