movement of children between group activities. Slip-n-Slide weights, chemical ice pack usage ("ziplock" baggies), Injury report form (printed 2 sided vs. 2 pages) attendance documentation.

Comments:
An in-person monitoring inspection was conducted today. All groups were in ratio during the inspection. Children were observed participating in a wide variety of activities. Staff records, emergency evacuation drills, emergency supplies, first aid supplies, medications were all reviewed. See Violation Notice for non-compliances. Standard #: 22.1-289.035-B-2
Description: Based on a review of records, one staff record did not contain documentation of a nationwide fingerprint search result prior to employment.
Evidence:
1 of 4 staff records reviewed did not have a fingerprint result on file prior to being employed. The results are dated 6/8/22. (Staff B - Date of Hire (DOH) 6/6/22.

Plan of Correction: In the future, we will make sure that all staff have completed these steps before their hire date.

Standard #: 8VAC20-770-60-B
Description: Based on a review of records, one staff record does not contain an accurate Sworn Disclosure Statement.
Evidence:
1 of 4 staff records reviewed had a Sworn Disclosure Statement (SDS) that did not accurately reflect all residences outside of Virginia in the last 5 years. (Staff D)

Plan of Correction: Staff member will complete a new Sworn Disclosure including other states lived in. In addition, we will run a search in the additional state for this staff member.

Description: Based on a review of records, one staff record did not contain documentation of 2 references prior to employment.
Evidence:
1 of 4 staff records reviewed contained 1 of 2 required references properly documented. The reference taken over the phone did not include the signature of the person making the phone call. (Staff C)

Plan of Correction: For this individual, we did find the physical copy of an additional reference. For any additional staff in the future, we will be sure to include signed references for staff before their hire date.

Description: Based on a review of records, child records do not contain all required components. Evidence:
2 of 3 child records reviewed did not contain an allergy action plan for a diagnosed food allergy. (Child 1,2)

Plan of Correction: Parents will be contacted to complete or provide the FARE form/send in the completed form from their doctor. In addition, any new incoming campers will be required to include the FARE form(or equivalent form) on their first day and when turning in medications for diagnosed food allergies.

Evidence:
1 of 4 staff records reviewed contained a TB screening result dated 7/1/22. (Staff C: DOH: 6/16/22

Plan of Correction: In the future, we will make sure that all staff have completed these steps before their hire date.

Description: Based on a review of records, staff records do not contain documentation of orientation.
Evidence:
1 of 4 staff records reviewed did not contain documentation of orientation dared in the allowable time frame. The form used for orientation was not available in 7/2021. (Staff A)

Plan of Correction: I will locate the orientation form from last year. In addition, we will continue to include this for each new staff member as well.
