The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 03/28/2019 regarding allegations of supervision, injury reporting, and ratio. The inspection was initiated on 04/01/2019 and completed on 04/24/2019. On 04/01/2019, the inspector was on site from 11:48am to 2:57pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The inspector completed additional interviews on 04/24/2019, the inspector was on site from 11:30am to 12:50pm. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
