Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Childcare Centers was conducted on 5/3/19 from 11:35 a.m. to 2:00 p.m. At the time of arrival the following staff to child ratios were observed: 9 children, two years old, with 2 staff 8 children, two years old, with 1 staff 9 children, three years old, with 1 staff 8 children, three years old, with 1 staff 7 children, three years old, with 1 staff 7 children, four years old, with 1 staff 6 children, four years old, with 1 staff 9 children, four years old, with 1 staff 9 kindergarten children with 1 staff Classrooms were inspected, staff/child observations were made and children were observed playing outdoors, transitioning to lunch and resting quietly on cots at nap period. Restroom procedures, hand washing and table sanitizing were also observed. Twenty one (21) - all - staff records were reviewed, handbooks and policies were reviewed and staff was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space were inspected. The administrator reports that the program does not transport children in a vehicle but hires a bus company and driver when field trips are taken. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Standard #: 22VAC40-191-40-D-2
Description: Based on a review of records and interview, the provider did not obtain a signed and dated Sworn statement or Affirmation for each employed staff before employment or commencement of service at the facility. Evidence: Twenty one staff records were reviewed. The records for staff #8, #10, #16 and #21 did not contain a signed and dated Sworn statement or Affirmation form. None of the records for 21 staff currently working in the preschool contained results of a Search of the Central Registry. Staff #1 confirmed that each of these staff are currently employed in the program. She stated that she was under the misunderstanding that the new finger print based background check had replaced all of the other background checks and were no longer required. Therefore, these reports were not obtained / maintained.

Plan of Correction: Staff #1 stated that she will have all staff complete forms and have them notarized and mailed off by Monday 5/6/2019. Staff #8, #10, #16 and #21 will complete, sign and date a Sworn statement or Affirmation form by Monday 5/6/2019.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on a review of records and interview, the religious exempt preschool program has not had all employed staff submit to fingerprinting and provide personal descriptive information described in subdivision B 2 of ? 19.2-392.02 of the code of Virginia in order to obtain a National Finger print based background check. Evidence: Twenty one (all) staff records for the program were reviewed. The records for staff #20 and #21, who began employment in January 2019 are who are currently working in the preschool, did not contain results of a National Finger print based background check. Staff #1 confirmed that each of these staff are currently employed in the program. She stated that she was under the misunderstanding that the new finger print based background check had replaced all of the other background checks and were no longer required. Therefore, these reports were not obtained / maintained.

Plan of Correction: Staff #1 set an appointment , during the inspection, for staff #20 and staff #21 to have fingerprints taken on Monday 5/6/2019. The results of these background checks will be forwarded to the agency within 10 days.
