A complaint was received in the licensing office on March 29. 2023 regarding household member providing care for children and did not have background check results. On March 31, 2023 an on-site investigation was conducted. Interviews were conducted and observations were made.

Description: Based on interviews and a review of one household member's record, the provider did not ensure a household member was not alone with, in control of, or supervising one or more children, without first having completed any required background checks.
Evidence:
1.Household member moved in the home on March 23, 2023.
2. The record of Household Member 1 did not have documentation of results for a criminal history check, a search of the child abuse and neglect from Virginia (VA), or a sworn statement.
3. Interviews confirmed that Household Member has been left alone with children in care to supervise them while the provider prepared meals and completes other tasks in a different room.

Plan of Correction: A fingerprint appointment is scheduled for April 22 2023. The household member will be assisting the home. Central registry form was mailed about two weeks ago. Household member will complete sworn statement.
