Comments:
The Licensing Inspector for Fincastle Baptist Church conducted an unannounced RECDC Monitor Inspection on Tuesday, May 7, 2019. Licensing Inspector arrived & started inspection at 12:00 pm and concluded at 2:20 pm. Upon arrival, there were a total of fourteen (14) children ages 3-5 years old under the direct supervision of two (2) staff members. The Licensing Inspector reviewed information for 5 enrolled children & information for 5 staff members. LI Anderson conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Director on May 7, 2019. Standard #: 54.1-3408-O
Description: Based on inspection & discussion with the Director, the Center failed to have a staff person available who has received the required Medication Administration Training to administer prescribed medication. EVIDENCE: Center administers prescribed medication; the Licensing Inspector observed prescribed emergency medication at the center for a child currently enrolled in the program. Center does not have a staff person available who has received the required Medication Administration Training (MAT) required when administering prescribed medication.

Plan of Correction: At least 1 staff member will receive the required MAT certification to administer prescribed medication.

Description: Based on inspection, the Center failed to disclose in writing to the parents or guardians of the children in the center and the general public, the qualifications of the personnel employed. EVIDENCE: Qualifications of personnel were not disclosed in writing to the parents/general public.

Plan of Correction: Director will add staff qualification information to the parent/staff handbooks.

Description: Based on review of center information, the Center failed to provide a written statement regarding physical facilities, health requirements for the staff and public liability insurance to the parents/guardians of the children in the center and made available to the general public. EVIDENCE: A written description of the center's physical facilities, information regarding the center's health requirements for staff and information regarding the center's public liability insurance were not included in the parent handbook nor posted.

Plan of Correction: The Director will add required information to the parent and staff handbooks.
