Comments:
A monitoring inspection was conducted on July 18, 2023 from 1:05 P.M.-4:00 P.M. There were 286 children present, ranging in ages from 4 years 11 months to 12 years of age, with 60 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 11 child records and 9 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of staff records, the center failed to a obtain updated background checks for employees every five years.

Evidence:
1. Staff #3's central registry finding on file was dated 07/13/2018. A new one was required by 07/13/2023. A request for the background check had not been sent off.
2. Administration verified the date on the central registry finding, and that a new one had not been sent off.

Plan of Correction: Administration sent the request off and sent the result to the Licensing Inspector on 7/20/2023.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of medications and children records, the center failed to ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction was in the child's record.

Evidence:
1. Child #2 had two emergency medications, (Epi-Pen and Children's Benadryl), on site. There was not a completed written care plan in the record. The physical stated the child was allergic to egg, coconut, tree nut, sunflower, and dairy.
2. Child #3 had two emergency medications, (Epi-Pen and Zyrtec Children Allergy Medication), on site. There was not a completed written care plan in record. The physical stated the child was allergic to cashews and pistachios.
3. Administration verified that there was not a written care plan in the records.

Plan of Correction: Administration contacted the parents while the Licensing Inspector was on site and requested the written care plan. One was received before the inspector left the inspection.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In Locker 121 in the pool area there was a bottle of Wasp and Hornet Spray. The locker door was opened, and there were children in the pool area.
2. The manufacturer's label stated "keep out of reach of children, caution, and hazardous to humas and domestic animals."

Plan of Correction: Administration went and removed the spray from the locker. Administration will remind all staff that hazardous materials must be locked.
