An unannounced monitoring inspection was conducted on 5/9/19 from 10:00 am until 1:00 pm to review the religious exempt requirements. At the time of the inspection 7 children were present with two staff. The sample size consisted of five children's records and four staff's records. Children and staff were observed during free indoor play, educational and artistic activities, hand washing, bathroom break, lunch, TV time, transitions, and behavioral guidance. Standard #: 63.2(17)-1720.1-A Description: Based on record review and interview, the center failed to have staff complete a sworn statement every five years effective 9/1/17.
Evidence:
1. The records for all four staff were reviewed. Staff 1 has a sworn statement dated 9/4/09 and staff 2 has a sworn statement dated 10/26/09.
2. The director stated they have not updated the sworn statements for staff 1 and staff 2.Plan of Correction: Both staff updated their sworn statements during the inspection. In the future the director will ensure sworn statements are updated every five years.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and interview, the facility failed to ensure a fingerprint-based criminal history check determination letter was obtained prior to 9/30/18 for staff hired before 1/22/18. Evidence: 1. The records of all four staff were reviewed. No record contained a fingerprint-based criminal history check determination letter. All four staff were hired prior to 1/22/18. 2. The director stated all staff got their fingerprints done prior to 9/30/18 but she never got results. She did not follow-up to see why she did not get results. She stated she assumed they would only get results if something was found. Plan of Correction: The director will contact the Office of Background Investigations to follow-up on the fingerprint background checks. If they do not have them all staff will be required to complete the fingerprint background checks again. In the future the director will ensure fingerprint results are obtained prior to hire.
