Comments:
An unannounced monitoring inspection was conducted on May 9, 2019. Licensing inspector arrived at 9:50 am and departed at 12:30 pm. At the time of the tour there were four children present with one staff member. Additional persons were in the building for administrative and support purposes. Children were observed playing in centers. The center has access to a public school playground directly across the street for outdoor play. The center has a total enrollment of four children. The sample size was two staff records and four children's records. The results of this inspection will be reviewed with the Licensing Administrator.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, in two of two staff records reviewed, the center did not ensure that staff have a sworn statement or affirmation completed before employment and results of a search of the central registry within 30 days of employment. Evidence: During the inspection conducted on May 9, 2019, there was not a completed sworn statement or affirmation or results of a search of the central registry for Staff #1 (date of hire 4/9/18) and Staff #2 (date of hire October 2018).

Plan of Correction: Both of the documents have been printed and filled out and we hope to have this done by Monday.

Description: Based on observation and inspection of the facility, the center had not disclosed in writing to the parents or guardians of the children in the center the qualifications of the personnel. Evidence: During the inspection conducted on May 9, 2019, the center had not disclosed in writing to the parents or guardians of the children in the center of the qualifications of their staff.

Plan of Correction: We will see if the staff member has created the pamphlet and if she has we will generate to the parents.

Standard #: 63.2(17)-1716-A-4
Description: Based on record review, in one of two staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision. Evidence: During the inspection conducted on May 9 2019, there was no documentation that Staff #2 (date of hire October 2018) had been certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision.

Plan of Correction: The staff member will get a physical.

Standard #: 63.2(17)-1716-A-6
Description: Based on observation and inspection of the facility the center did not ensure that all aspects of the child day center's operations are described in a written statement provided to the parents or guardians of the children in the center and made available to the general public. Evidence: During the inspection conducted on May 9, 2019, the following information had not been provided in writing to the parents or guardian of the children in the center and made available to the general public: physical facilities, enrollment capacity, health requirements for the staff and public liability insurance.

Plan of Correction: We will see if the staff member has created the pamphlet and if she has we will generate to the parents.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, in one of two staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results. Evidence: During the inspection conducted on May 9, 2019, there was no documentation of fingerprint results for Staff #2 (date of hire October 2018).

Plan of Correction: We will make sure that staff member gets the fingerprint check done.
