Comments:
An initial subsidy health and safety inspection was conducted on May 15 2023 between 11:00am and 12:00pm. There was one child in the direct care of one caregiver. The child was observed napping during the inspection. A walkthrough of the home was conducted. A review of household records, caregiver records, child records, attendance records, emergency response documentation, and the emergency supplies was conducted. Description: Based on record review, the vendor did not have all required documentation for child records.

Evidence: Child #1 record did not contain an emergency contact form or a completed proof of identity form.

Description: Based on record review, the vendor did not have a written preparedness plan that addresses caregiver responsibility and home readiness with respect to lockdown and shelter-in-place procedures.
