Discussion: - All background checks (sworn statements, fingerprints and search of the central registry (CPS)) must be updated every five (5) years. - Although there is a section in your parent handbook titled insurance (p 5), there is no information in this section regarding insurance.

Comments:
Licensing inspector arrived at 10:45 AM and departed at 12:15 PM to conduct a religiously exempt code compliance inspection. At the time of the inspection, there were 21 children (ages two through five years) in care in the primary classroom. There was private school being held in the adjacent building. Children were observed celebrating a child's birthday and playing outside.

Description: Based on a review of staff records, it was determined that the facility did not ensure that all staff had a search of the central registry (CPS check) on file within 30 days of employment. Evidence: The following staff did not have completed CPS checks on file: - staff #3 - staff #8

Plan of Correction: Center management will ensure that all staff complete CPS checks and that results are placed in the record

Standard #: 63.2(17)-1716-A-6
Description: Based on review of the handbook received on site during the inspection, it was determined that the facility did not ensure that all required aspects of the center's operations were described to the parents in writing. Evidence: The following required items were not in the parent information: - Health requirements for staff - Public liability insurance

Plan of Correction: Information will be updated in the handbook for next year.

Standard #: 63.2(17)-1716-B-4
Description: Based on review of records and interview with staff, it was determined that the facility did not ensure that the center established and implemented procedures to ensure that a person trained and certified in first aid is present at the center whenever children are present. Evidence: Staff #5 stated during the inspection that first aid certification for the staff had expired.

Plan of Correction: Center management will ensure that first aid training is updated and someone with current training is always present at the facility.

Description: Based on review of documents at the center, it was determined that the facility did not ensure that all background documents were updated every five years. Evidence: The following staff had a search of the central registry (CPS check) that was over five years old: - staff #2 (last done 9/6/11) - staff #4 (last done 8/23/13) - staff #6 (last done 9/1/10) - staff #7 (last done 9/6/130

Plan of Correction: Center management will ensure that all CPS checks are updated and that in the future all background checks are updated every five years.

Description: Based on review of center documents, it was determined that the facility did not ensure that all staff had completed fingerprints on file as required. Evidence: The following staff did not have results of fingerprinting on file: - staff #3 - staff #8

Plan of Correction: Center management will ensure that all staff complete fingerprinting and that results are placed in the records.
