Description:	Based on a review of records, one staff record did not contain documentation of a negative TB screening result completed within the last 30 calendar days of the date of employment. Evidence: Staff #1's record contained documentation of a negative screening result that was dated more than 30 calendar days of the date of employment. Result is dated 8/9/22. Date of Hire was 7/10/23.
Plan of Correction:	Moving forward we will ensure that the TB results are within 30 days prior to the start date.

Description:	Repeat Violation Based on a review of records and Director's statement, one staff record did not contain documentation of completion of the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Evidence: Staff #4's record did not contain documentation of a completion of the course within 90 day of the date of employment. The Director state they were working on competing the course. (Date of Hire 6/19/22.)

Description:	Based on a review of records, staff records did not contain documentation of CPR certification from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration. Evidence: The CPR certifications available for review were from an online course that has no in-person competency demonstration. (Staff # 1,2,3,4)
Plan of Correction:	Moving forward, we will confirm that all training contains an in person component and from an acceptable provider.
