Licensing Inspector arrived at 9:45 and departed at 10:45 AM to conduct a religiously exempt child day care code compliance inspection. At the time of the inspection, there were seven children in care with two staff. Children were observed participating in free play. The sample size was five staff records.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of five staff records, it was determined that the facility did not ensure that all staff had a sworn statement or affirmation on file before employment and a search of the central registry (CPS check) within 30 days of employment. Evidence: 1. The following staff did not have a signed sworn statement or affirmation on file: - staff #3 - staff #5 2. The following staff did not have a completed CPS check on file: - staff #2 - staff #3 - staff #5 Plan of Correction: All sworn statements will be signed and placed in records. All CPS checks will be submitted and results placed in records.
Standard #: 63.2(17)-1721.1-A Description: Based on review of five staff records, it was determined that the facility did not ensure that background checks were updated every five years. Evidence: The most recent search of the central registry (CPS check) for staff #4 was dated 8/12/11 and was, therefore, over five years old. Plan of Correction: Updated CPS check will be submitted and results placed in the record.
