An unannounced monitoring inspection was conducted on 5/8/19 and a review of staff records on 6/3/19. A violation notice was completed regarding the violations found during the inspection. Please remember to contact the inspector with information that the violations are being corrected within 10 days of receipt of this notice. There were 14 children in care with two staff present during the inspection. The children practiced their end of the year program during the inspection. We discussed that in order to administer medications, including Epi-pens and inhalers a staff person must be MAT trained and certified. Should you have any questions please do not hesitate to contact me. Inspection began: 9:40 am ended 11:20 am.

Standard #: 22VAC40-191-40-C-2 Description: Based on review of staff records on the center did not have the required background check information on each staff. Evidence: 1. Staff # 1-5 do not have a central registry check or sworn statement or affirmation on file. The staff have been employed 2006 through 2016. Plan of Correction: The staff will complete these forms and ensure that everyone is in compliance before the new year begins in August, 2019.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records no one has a fingerprint-based criminal history check on file. Evidence: 1. Staff #1-5 do not have a fingerprint-based criminal history check on file. Plan of Correction: All staff will submit to a fingerprint based criminal history check prior to the center opening in August, 2019.
