An announced subsidy health and safety inspection was conducted on August 31, 2023 beginning at 1:03pm and ending at 2:35pm. There were two children in attendance and directly supervised by the vendor. Children were observed playing with age-appropriate toys and sleeping. The vendor's record, training certificates, daily attendance, child information/records, emergency procedures, physical plant and first aid supplies were reviewed. No medication on sight. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Standard #: 8VAC20-790-190-B Description: Based on record review, the provider failed to ensure all required information was documented in each child's record.
Evidence:
1. The record for Child # 3 and Child #4 did not have the name, address and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
2. The record for Child #1 and Child #2 did not have the name, address and telephone number for each custodial parent's place of employment.
3. The record for Child #1, #2, #3 and #4 did not have written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.Plan of Correction: I will have the parent provide the information today. I will get all the information in the future.
