Comments:
An unannounced monitoring inspection was conducted today, June 4, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 2:00pm and concluded at approximately 4:00pm. Today twenty-two preschool children were in attendance being supervised by four staff with the correct child/teacher ratio. Nine school age children arrived later in the inspection and were supervised by one staff. Administrative records, all areas in the center used by children, and common areas were inspected. Note: A follow up inspection was conducted on 06/06/19 to review staff records only.

Violations:
Standard #: 63.2(17)-1716-A-6
Description: Based on the visual inspection of the facility and administrative records, the required written Code of Compliance Statements were not available to review. Evidence: During the visual inspection of the facility and review of administrative records, the following written Code of Compliance Statements were not provided or available to review: 1. Written notice to parents or guardians of staff qualifications. 2. Written statement provided to parents or guardians and made available to the general public regarding physical facilities. 3. Written statement provided to parents or guardians and made available to the general public regarding health requirements for staff. 4. Written statement provided to parents or guardians and made available to the general public regarding possession of liability insurance. *Inspector provided a written Public Disclosure Statement template on site.

Plan of Correction: Per administrator: We have them, however, they are not present. I will make sure they are kept in the center.

Standard #: 63.2(17)-1716-B-2
Description: Based on the supervision of school age children, the appropriate supervision was not maintained. Evidence: During the inspection of the school age classroom with nine children, staff was observed leaving the classroom several times for up to approximately twenty five minutes. As a result, the appropriate supervision of the children was not maintained.

Plan of Correction: Per administrator: I will have a meeting with all staff regarding supervision and will also hire a floater to supervise the children if the teacher leaves the classroom.

Standard #: 63.2(17)-1716-B-4
Description: Based on the inspection of administrative records, there were no staff with current first aid certifications.

Evidence:
During the inspection of four staff records, it was discovered that all four staff were certified in first aid in March 2017. These certifications expired in March 2019 with no further updates in the record. Religious exempt requirements mandates at least one person to be present at all times with current first aid certification.

Plan of Correction: Per administrator: We are all scheduled to get the updated certification training on June 15, 2019.
