Comments:
An unannounced monitoring inspection was conducted on 6/25/19 at Stephens City United Methodist Church from 10:35 am until 1:00 pm to review the religious exempt requirements. At the time of the inspection 37 children were present with 10 staff. The sample size consisted of five children's records and five staff's records. Children and staff were observed during free indoor and outdoor play, diaper changing, bottle feeding, transitions and behavioral management. Violations were found during this inspection and are documented on the violation notice. If you have questions or concerns contact the licensing inspector at (540) 292-5933 for further assistance.

Description: Based on record review and interview, the center failed to ensure all staff completed a central registry record check within 30 days of employment for two out of five staff. Evidence: 1. Staff 3's start date was 7/16/18 and the central registry search is dated 3/20/19. Staff 5's start date was 7/30/18 and the central registry search is dated 9/14/18. 2. The director stated they had a change in who was completing the paperwork and information was not communicated.

Plan of Correction: The issue has been resolved and in the future central registry searches will be completed within 30 days of employment.

Description: Based on record review and interview, the center failed to ensure three out of five staff hired after 1/22/18 had a fingerprint-based criminal history check determination letter prior to the first day of employment. Evidence: 1. Staff 1's start date was 2/28/18 and the fingerprint results are dated 3/7/18. Staff 2's start date was 11/26/18 and the fingerprint results are dated 1/29/19. Staff 5's start date was 7/30/18 and the fingerprint results are dated 8/2/18. 2. The director verified the start dates and dates of fingerprint results.

Plan of Correction: In the future all potential staff will be required to obtain fingerprint-based criminal history checks prior to the first day of employment.
