Comments:
An unannounced monitoring inspection was conducted Friday, June 28, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 9:30 AM and concluded at approximately 2:10 PM. The center was in compliance with the required staff to child ratio. The following was observed for a total of 136 children in the building:
Infants I - 5 children with 2 staff
Infants II - 2 children with 2 staff
Toddlers - 10 children with 3 staff
Twos - 11 children with 2 staff
Twos and Threes - 13 children with 2 staff
Threes ? 15 children with 2 staff
Threes and Fours ? 12 children with 2 staff
Pre-K - 13 children with 2 staff
Pre-K - 14 children with 2 staff
School Age - 41 children with 4 staff

During the inspection, children were observed creating confetti balls and tie-dyed scenes with coffee filters, playing in dramatic play, building with legos and blocks, putting on a puppet show, watching a magician, playing with toys, playing outside, taking bathroom breaks and washing hands, eating lunch and resting at naptime. The following items were reviewed during the inspection: 10 children's records, 10 staff records. All areas of the center used by children were inspected. Policies and procedures were discussed. Written notification to parents and the public was reviewed. Violations were cited during the inspection. See violation notice for additional details.

The provider must send documentation to the licensing inspector that the background checks have been requested no later than 10 days following this notification dated June 28, 2019.

Standard #: 22VAC40-191-40-D-2
Description: Based on review of 10 staff records and interview, the center did not ensure that one staff had a search of the central registry within the first 30 days of employment or volunteering. Evidence: 1. The record for Staff #5 (hire date 02/24/14) did not contain central registry results. 2. Staff #11 confirmed this while the licensing inspector was on site.

Plan of Correction: Ensure new employees have central registry search within first 30 days moving forward.

Standard #: 63.2(17)-1716-B-5
Description: Based on review of 10 children?s records and interview, the center did not establish and implement procedures to ensure that two children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.
Evidence:
1. The record for Child #2 (start date 03/04/19) contained documentation of immunizations dated 05/08/19.
2. The record for Child #8 (start date 06/17/19) did not contain documentation of immunizations.
3. The parent handbook states the immunization records must be submitted prior to child?s first day at the center.
4. Staff #11 confirmed this information while the licensing inspector was on site.

Plan of Correction: Ensure center obtains immunizations prior to start date.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of 10 staff records and interview, the center did not ensure two staff members undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter. Evidence: 1. The record for Staff #6 (hire date 01/06/12) contained documentation of a central registry finding dated 01/20/12. 2. The record for Staff #8 (hire date 09/03/13) contained documentation of a central registry finding dated 10/01/13. 3. Staff #11 confirmed this information while the licensing inspector was on site.

Plan of Correction: Ensure staff background checks are completed prior to employment start date and then every 5 years thereafter.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of 10 records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff member prior to employment. Evidence: 1. The record for Staff #7 (hire date 09/10/18) contained satisfactory results of the finger-print based national criminal background check dated 09/20/2018. 2. Staff #11 confirmed this information while the licensing inspector was on site.

Plan of Correction: Ensure fingerprint based check is complete before start date.
