An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios (grouping of children), CPR, out of state searches, and accessing the department's website for memos, forms, and additional information.
- Comments:

An unannounced monitoring inspection was conducted today, June 25, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 1:15pm and concluded at approximately 3:35pm. Today 14 children (ages 3yrs-5yrs) were in attendance being supervised by four staff with the correct child/teacher ratios. Administrative records, children's classrooms, restrooms, common areas, and playground were inspected. See attached notice for the violation found during this inspection. Standard #: 63.2(17)-1716-A-4 Description: Based on the inspection of administrative records, the required staff health information form has not been updated.
Evidence:
During the inspection of five staff records, it was discovered staff #2 has a staff health form dated 5/22/18. This form expired on 5/22/19 with no further updates in the record. Religious exempt requirements stipulate for each person in a supervisory position to be certified by a practicing physician to be free from any disability which would prevent him from caring for children under his supervision.Plan of Correction: Per director: I will have the staff to obtain an updated staff health information form from the physician.
