Licensing Inspector arrived at 10:30 and departed at 11:45 AM to conduct a religiously exempt code compliance inspection. At the time of the inspection, there were three three-old child children in care with two staff members. The children were observed participating in center base free play and watching a movie.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of two staff records, it was determined that the facility did not ensure that each staff member had a sworn statement or affirmation prior to employment and a search of the central registry within 30 days of employment.
Evidence:
1. Neither of the two staff members on duty at the time of the inspection had sworn statement or affirmations on file.
2. Neither of the two staff members on duty at the time of the inspection had a completed search of the central registry on file. Staff #1 stated that they had both been employed over 30 days.Plan of Correction: Both staff signed sworn statements during inspection and will complete and submit search of the central registry checks the next day.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of two staff records, it was determined that the facility did not ensure that each staff member had submitted to fingerprinting as required prior to working with children.
Evidence:
Neither of the staff on duty at the time of the inspection had completed fingerprinting through the approved state vendor.Plan of Correction: Both staff made appointments for 7/3/19 during the inspection.
