An unannounced monitoring inspection was conducted Wednesday, July 10, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:25 AM and concluded at approximately 1:00 PM. During the inspection, there were a total of 26 children being supervised by five staff members. Children were observed playing outside, washing hands and taking bathroom breaks, eating snack and lunch, learning about the different food groups and resting at naptime. All areas of the center were inspected including the classroom, hallways, bathroom and outdoor play area. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. A violation was cited during the inspection. Standard #: 63.2(17)-1716-A-3-e Description: Based on observation and interviews, the center did not ensure that the ratio of one staff member for every 20 children from ages five years to nine years was met. Evidence: 1. From approximately 10:25 AM until 10:40 AM, the Toddlers (four children present), Twos (four children present), Threes (four children present, Fours (five children present) and school age children (nine children present) for a total of 26 children were outside on the playground with three staff members. Two staff members stated Staff #1 was inside with a child that was sick and Staff #2 was inside looking for a key. Staff #2 was the assigned school age teacher. Staff #2 returned to the playground at approximately 10:40 AM. The ratio required one staff member be present for the children five years to nine years of age. Plan of Correction: Teachers will call the administrator for help on the playground during potty time. When there is no administrator, an extra staff person will be scheduled.
