Comments:
Licensing Inspector arrived at 10:00 and departed at 11:30 AM to conduct a subsidy health & safety inspection in conjunction with a religiously exempt code compliance inspection. At the time of the inspection there were 50 children ages infant through school age in care. Children were observed participating in free play activities and eating lunch. All staff records and four children's records were reviewed.

Violations:
Standard #: 22VAC40-665-520-B
Description: Based on review of four children's records reviewed, it was determined that the vendor did not ensure that all required elements were present in each child's record.

Evidence:
The records for child #3 and#4 did not include addresses for the emergency contact person.

Plan of Correction: Center director will contact parent to obtain emergency contact person address and place in the folder.

Standard #: 22VAC40-665-540-A
Description: Based on review of 14 staff records, it was determined that the vendor did not ensure that all staff had a tuberculosis (Tb) screening on file prior to beginning work with the children that was no more than 30 days old.

Evidence:
Staff #13 and #14 were both identified as employees who were working with the children. Neither of these staff had documentation of Tb screenings on file.

Plan of Correction: Center director will ensure that staff obtain Tb screenings and place results in the records.

Standard #: 22VAC40-665-750-A
Description: Based on review of medication on site at the facility, it was determined that the vendor did not ensure that any medication that could be administered to a child had parental authorization.

Evidence:
There was a ProAir inhaler on site for child #5 that did not have parental authorization.

Plan of Correction: Center director will contact the parent to obtain authorization for the medication.
