Comments:
An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted on July 19, 2019 from 9:15 a.m. to 12:15 p.m. 26 children, ages 3 months through 5 years, were under the care and supervision of 5 staff. Children were observed being fed by staff, playing with toys, doing circle time, and watching an age appropriate movie. Center documents and nine staff records were reviewed. Standard #: 22VAC40-191-40-D-2
Description: Based on review of staff records, the center failed to ensure central registry background checks were conducted within 30 days of employment, . Evidence: Staff C, hired 5/20/19, did not have documentation of a central registry background check results in the record.

Plan of Correction: The center will request the central registry background check for the staff and place it in her record.

Standard #: 63.2(17)-1716-A-3-c
Description: Based on observation, the center did not maintain the required ratio of one staff member to eight children from ages 24 months to 36 months of age. Evidence: At approximately 9:30 AM, there were 9 children present in the Young Three class (three and four years of age) with one teacher.

Plan of Correction: The ratio requirements recently changed and the center just became aware of it. The center corrected the ratio for the three year old classroom immediately.
