Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on 7/24/19 from12:00 p.m. to 1:15 pm. At the time of arrival the following staff to child ratios were observed: 5 infants, 7 to 15 months old with 2 staff 5 children, 16 to 23 months old, with 2 staff 6 children, two years old, with 1 staff 6 children, three years old, with 1 staff 7 children, three years old, with 1 staff 4 children, four years old, with 1 staff 6 children, five to 7 years old, with 1 staff The records sample size consisted of 13 staff records. Classroom observations were made and children were observed eating lunch and resting quietly on mats during nap period. Handbooks and posted information were reviewed and the center director was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space were observed. Staff report that the program does not transport enrolled children in a vehicle. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Standard #: 22VAC40-191-40-D-2
Description: Based on a review of records and interview, the religious Exempt Child Day Center was unable to present evidence that a search of central registry was obtained for all employed staff within 30 days of employment or commencement of service. Evidence: Thirteen staff records for staff currently employed by the center were reviewed. The records for staff #2, staff #5, staff #6, staff #8, staff #9 and staff #12 did not contain results of a central registry background check. Staff #3confirmed that this documentation was not present in the files or available and that all of these staff had been employed in excess of 10 years and were employed prior to staff #3 beginning as the Director for the program in 2009.

Plan of Correction: Staff #3 stated that she will have Central Registry Search requests completed by the end of the week for these 6 staff and will forward the results to the licensing inspector upon receipt.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of records and interview, the religious Exempt Child Day Center was unable to provide evidence that a search of the central registry had been conducted every 5 years following the intitial search of the central registry that was conducted at the time of each employed staff's hire. Evidence: The record for staff #3 whose date of hire was documented as September 2009 contained he most recent results of a Search of the Central Registry which was dated 11/4/09. The record for staff #11 whose date of hire was September 2011 contained he most recent results of a Search of the Central Registry which was dated 4/7/14. Staff #3 confirmed that she was unaware that Central Registry Searches had to be repeated after the initial Search at the time of hire and that more recent background checks had not been obtained for staff #3 and staff #11.

Plan of Correction: Staff #3 stated that she will have updated Central Registry Search requests completed by the end of the week for these 2 staff and will forward the results to the licensing inspector upon receipt.
