An unannounced Code compliance inspection was conducted today, 7/26/19. We reviewed the Code compliance requirements and spent time on the VDSS web site discussing background check requirements. A violation notice was completed regarding the violations found during today's inspection. There are seven children in care with five staff present. A review of staff and children's records was also conducted. Should you have any questions please do not hesitate to contact your inspector. Inspection began: 10:10 am and ended 12:20 pm.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of staff records and interviews the center did not have the required background information on file. Evidence: 1. Staff #1-8 do not have a sworn statement or affirmation form on file. 2. Staff #1-8 do not have a central registry check on file. Plan of Correction: The director will have all the staff sign these forms. The process for the registry checks will begin Monday morning. Documentation will be provided within 10 days that this process has begun.
Standard #: 63.2(17)-1716-A Description: Based on review of written disclosures to parents or guardians the center did not have all the informtion listed. Evidence: 1. The written information provided to the parents/guardians did not include; qualifications of personnel and the fact that the center is exempt from licensure is not posted in a visible location. Plan of Correction: The director will post this information. This information will be added to the information provided to the parents.
Standard #: 63.2(17)-1716-A-6 Description: Based on review of written information provided to the parents the center did not include all the required information. Evidence: 1. The written information provided to the parents/guardian does not include: enrollment capacity, health requirements for staff and public liability insurance. Plan of Correction: The director will add this information to the parent handbook.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records and interviews with staff the center did not have all staff submit to fingerprint background checks. Evidence: 1. Staff #1-8 do not have on file a fingerprint-based criminal history check determination letter prior to the first day of employment or service. Plan of Correction: The director will begin the process for having staff submit to fingerprint background checks and will provide information to the LI within 10 days that this process has begun.
