Comments:
An unannounced monitoring inspection was conducted Friday, July 26, 2019 from approximately 11:15 AM until 12:45 PM and on Tuesday, July 30, 2019 from approximately 1:00 PM until 2:15 PM to determine the center's compliance with religious exempt requirements. On Friday, July 26, 2019 there were 18 children present, being supervised by four staff members. Children were observed playing outside, taking bathroom breaks and washing their hands, eating lunch and resting at nap time. All areas of the center were inspected including the classroom, hallways, bathroom and outdoor play area. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public were reviewed. Violations were cited during the inspection. The provider must send documentation to the licensing inspector that the background checks have been requested no later than 10 days following this notification dated July 30, 2019. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of five staff records, it was determined that the center did not ensure that all staff and volunteers had a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
1. The record for Staff #1 (hire date 08/2018), Staff #4 (hire date 7/11/18) and Staff #5 (hire date 5 /14/19 did not contain central registry results.
2. A staff member stated the requests were sent off and there has been ongoing communications with the agency.

Plan of Correction: We will have staff complete another central registry form and send it to the agency.

Standard #: 63.2(17)-1716-A-9
Description: Based on observation and interview, the center did not ensure compliance with all safe sleep guidelines recommended by the American Academy of Pediatrics.
Evidence:
1. Four infants were in Room 140. Staff #1 stated the infants will sleep in the reclining high chairs because the air conditioning was not working in the room where the cribs were located.
2. Child #5 (eight months old) was observed sleeping in a reclined high chair.
3. The Guide to Safe Sleep by the American Academy of Pediatrics states ?Place babies to sleep only in a safety-approved crib with a firm mattress and a well-fitting sheet?.

Plan of Correction: We moved the cribs to the classroom while the air conditioner was being repaired.

Standard #: 63.2(17)-1716-B-3
Description: Based on a review of records and interview, the center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings to exclude children as necessary. Evidence: The licensing inspector asked staff if the center had a procedure for ensuring that a daily simple health screening and exclusion of sick children is implemented. Staff stated that sick children are excluded, but that no staff had been trained on screening for illness at this point.

Plan of Correction: We will have staff take the online class that meets this requirement.

Standard #: 63.2(17)-1716-B-5
Description: Based on interview, the center failed to implement the procedure that ensures that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
Evidence:
The playground equipment, including slides, climbing wall and swings were placed on the bare ground. The playground equipment did not have cushioning material under it. Children were observed on the playground equipment during the inspection.

Plan of Correction: The playground area is going to be re-done. Until that time, children will play in the grassy field areas.
