Comments:
An unannounced monitoring inspection was conducted Monday, August 12, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 2:00 PM and concluded at approximately 4:00 PM. A total of 13 children were present during the inspection being supervised by three staff members. The required staff-to-child ratio was maintained.
Children were observed playing outside, arriving back from a field trip and eating popsicles. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play area. Five children?s records and three staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2
Description: Based on review of three staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
1. The record for Staff #1 (hire date 09/15) did not contain central registry results or a completed sworn statement.
3. Staff #1 was observed working alone with the children during the inspection.

Plan of Correction: We will look for the background checks and if they cannot be found we will request new ones.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of three staff records, two records did not contain the required updated background checks. Evidence: 1. The record for Staff #2 (hire date: 05/2009) contained a central registry finding dated 06/26/09 and a sworn statement dated 05/26/2009. 2. The record for Staff #3 (hire date: 08/2010) contained a central registry finding dated 02/22/10 and a sworn statement dated 07/21/2011. 3. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: We will update the required background checks.
