63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted on August 16, 2019 from approximately 9:10am-11:30am. There were two staff members present but there were no children present. One classroom, two restrooms and the cafeteria were inspected. Two staff records were reviewed. The center does not offer transportation.

Violations:
Standard #: 63.2(17)-1716-A
Description: Based on inspection of the facility, the center did not comply with information required to disclosed in writing to the parents or guardians of the children in the center and/or posted on the premises. Evidence: Staff confirmed that the center had not disclosed in writing to the parents or guardians of the children in the center the fact that it is exempt from licensure and the qualifications of the personnel. The center also had not posted the fact that it is exempt from licensure in a visible location on the premises.

Plan of Correction: We will have both of those items posted and inform parents within 10 days.

Standard #: 63.2(17)-1716-A-6
Description: Based on inspection of the facility, the center had not described all required aspects of the child day center's operations in writing to the parents or guardians of the children in the center and made available to the general public. Evidence: Staff confirmed that the center had not disclosed in writing the following information: physical facilities, enrollment capacity, health requirements for staff and public liability insurance.

Plan of Correction: The corrections will be noted in a document to the stakeholders within 10 days.

Standard #: 63.2(17)-1716-B-7
Description: Based on inspection of the facility, the center had not established and implemented procedures regarding reports of serious injury or death to children attending the child day center. Evidence: Staff confirmed that the center had not established and developed the following procedures: 1. All incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner. 2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. 3. Reports of deaths shall be submitted no later than one business day after the death occurred.

Plan of Correction: Principal will establish a policy of having a route for reporting serious injuries or deaths.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, in one of two staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results prior to employment or service. Evidence: Staff confirmed that the hire/start date for Staff #2 was 8/2/18 and the fingerprint results were dated 10/15/18.

Plan of Correction: There will be a policy written that anyone running our religious exempt program; the fingerprints will be on file before any training and official hire is established.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, in one of two staff records reviewed, the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: Staff confirmed that the center had not obtained an out-of-state child abuse and neglect search for Staff #2 (date of hire/start date 8/2/18) who indicated living outside of the state of Virginia within the last five years.

Plan of Correction: Principal will obtain the background check from Tennessee and it will be forwarded to the Licensing Inspector.
