An unannounced monitoring inspection was conducted on August 20, 2019 from approximately 9:00 am to 11:10 am to review religious exemption requirements. Five staff were present with twenty-three children. Eight staff records were reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on observation and review, facility did not ensure 1 of 8 staff have a completed sworn statement or affirmation prior to first day of employment. Evidence: The record for staff #5 did not have a completed sworn statement or affirmation. Staff #5 was observed working in the classroom. Program director stated today is the staff member's first day and she is working as a substitute. Plan of Correction: Sworn statement or affirmation was completed during inspection.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on observation and review, facility did not ensure 1 of 8 staff have a fingerpirnt-based criminal history check determination letter prior to first day of employment. Evidence: The record for staff #5 did not have a fingerprint determination letter. Staff #5 was observed working in the classroom. Program director stated today is the staff member's first day and she is working as a substitute. Plan of Correction: Staff #5 will be given information to make her appointment at the fingerprinting location to complete the fingerprint process.
