An unannounced monitoring inspection was conducted today, 9/9/19. Upon arrival to the center the children were busy working on preschool activities. Following this they went outside to the playground and then had lunch. Nap time was also observed. There are 16 children in care ranging in age from 3-5 years with three staff.
LI reviewed with the director the web site and the forms that are available there. We also discussed background checks.

Standard #: 22VAC40-191-40-D-2 Description: Based on review of staff records the center did not have all the required background checks information on file for each staff.
Evidence:
1. Staff #1 has a hire date of 8/8/19 and there is no Sworn Disclosure statement on file.
2. Staff # 1, 2, 4 do not have a central registry child protective services check on file. All three staff have been working more than 30 days.Plan of Correction: The director will ensure that all background checks are completed.
Standard #: 63.2(17)-1716-A Description: Based on observation and interviews the center did not have posted the fact the center is exempt from licensure.

Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records the center did not have all staff submit to fingerprint-based criminal history checks.
Evidence:
1. Staff #1-4 have hire dates more than 30 days and they do not have a fingerprint-based criminal history check determination letter on file.Plan of Correction: The director will have all the staff submit to a fingerprint background check.
