Comments:

An unannounced monitoring inspection was conducted Wednesday, September 11, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:00 AM and concluded at approximately 1:00 PM. A total of 55 children were present during the inspection and the following staff to child ratios were observed: Infants (Butterflies): 5 children and 2 staff Toddlers (Fireflies): 8 children and 2 staff Twos (Caterpillars): 11 children and 2 staff Threes (Busy Beetles): 9 children and 1 staff Fours and Fives (Butterflies): 22 children and 3 staff Children were observed playing organized games, watching children?s programs on T.V., washing hands and taking bathroom breaks, eating lunch and resting at nap time. All areas of the center were inspected including the classroom, hallways, bathroom and outdoor play area. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public were reviewed. Violations were cited during the inspection. Standard #:	22VAC40-191-40-D-2
Description:	Based on review of five staff records, the center did not ensure that all staff and volunteers had a sworn statement of affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering. Evidence: 1. The record for Staff #2 (hire date 01/22/2019 did not contain current central registry results. Staff #2 has left the facility and was re-hired 01/22/2019. The central registry results located in the record was dated 02/15/2013.
Plan of Correction:	We will the request the required background checks within the next 10 days. We will submit proof of the requested background check to the licensing inspector.
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on review of five records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check by 09/30/2018 for one staff hired prior to 1/22/2018. Evidence: 1. The record for Staff #4 (hire date 05/18/2015) did not contained satisfactory results of the finger-print based national criminal background check. Staff were required to obtain satisfactory results of the finger-print based national criminal background check sent from the Background Office of Investigations by 09/30/2018.
Plan of Correction:	The staff member had completed a finger-print criminal record search that we thought would suffice until it was time to update background checks. We will have the staff member make an appointment to complete the required background checks within the next 10 days. We will submit proof of the requested background check to the licensing inspector.
Standard #:	63.2(17)-1720.1-B-3
Description:	Based on the review of the five staff records, the center did not request a search of out-of-state founded complaints of child abuse and neglect within the first 30 days of beginning employment for one staff member who has lived outside of Virginia within the last five years. Evidence: The record for Staff #3 (hire date 09/19/17) indicated the staff member has lived outside of Virginia within the last five years. The record did not contain documentation of a request for founded complaints of child abuse or neglect from the state the staff member indicated she had lived in within the last five years
Plan of Correction:	We will request the out of state check for founded cased of child abuse and neglect within the next 10 days. We will submit proof of the requested background check to the licensing inspector.
