Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on September 13, 2019 from 11:00 a.m to 12:30 pm. At the time of arrival the following staff to child ratios were observed:
11 children, three to four years old, with 2 staff
16 children, three years old, with 2 staff
8 children, two years old, with 2 staff
3 children, I year old, with 2 staff
Classroom observations were made and children were observed washing hands, eating lunch which the children bring from home, engaged in gross motor play, and being picked up by parents at dismissal. Eight staff records were reviewed, handbooks and posted information were reviewed and the center program director was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space, were observed.
Staff report that the program does not transport children in a vehicle while in care.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on a review of records and interview, the religious exempt center was unable to demonstrate that a search of the central registry was conducted and results obtained within 30 days of employment for all staff.
The staff records for all (9) staff who are currently employed in the exempt program were reviewed. The record for staff #3 who has been employed at the center since February 2011, contained an outdated Search of the Central Registry which was dated 9/30/04.
The record for staff #7 who has been employed at the center since July 2014, did not contain the results of a Search of the Central Registry.
Staff # 8 confirmed that she was unable to locate this documentation during the inspection. These staff were hired prior to Staff #8 beginning employment as Program Director and it was an oversight that these reports were not noted as missing.

Plan of Correction: The program director stated that she would have forms completed today to have a Search of the central registry completed for both staff. Results will be placed in the staff's records upon receipt and a copy will be forwarded to the licensing inspector at that time.
