Comments:
On July 11, 2019 the Department received a complaint for an incident that occurred on June 28, 2019 regarding supervision. The licensing inspector and a representative from a local agency were on site conducting an unannounced inspection on July 17, 2019 from approximately 1:00 PM until 3:20 PM and on August 22, 2019 from approximately 10:00 AM until 10:30 AM. While on site, interviews with staff were conducted, observations were made and documentation was reviewed. Based on the information gathered during the investigation, the preponderance of evidence supports the allegation, therefore the complaint is valid. In addition to the original complaint, there was also a violation found during the inspection regarding safe sleep. See the violation notice for more details.

Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The provider?s responses for the ?plan of correction? were not received as of 09/16/2019 and will not appear on this Violation Notice.

Description: Based on observation, interview and documentation review, the center did not ensure compliance will all safe sleep guidelines recommended by the American Academy of Pediatrics.

Evidence:
During the July 17, 2019 inspection, Child B (10 months of age) was observed on her stomach, sleeping in a crib. The staff in the Infant room stated Child B was placed on her stomach in the crib to sleep. The American Academy of Pediatrics? A Child Care Provider?s Guide to Safe Sleep states ?Back to sleep for every sleep. To reduce the risks of SIDS, infants should be placed for sleep in a supine position (completely on the back) for every sleep by every caregiver until 1 year of life.?

Description: Based on staff interviews and documentation review, the center did not establish and implement procedures for appropriate supervision of all children in care, including daily intake and dismissal procedure to ensure safety of children.
Evidence:
During the July 17, 2019 and August 22, 2019 inspections, the following was determined:
1. Four staff stated on June 28, 2019, between approximately 7:30 AM to 8:00 AM, Child A (two years of age) was left on the center?s bus.
2. Staff #1 reported on the morning of June 28, 2019, he picked up children on his morning bus run and arrived at the center at approximately 7:30 AM. Staff #1 stated "Some of the older children on the bus where trying to unbuckle and hold the car seats that the babies were in when the bus was parked in front of the center. I was out of sorts. I just didn?t follow through with what I normally do because it scared me. I went and parked the bus in the parking lot and I left. [Staff #2] called me and said [Staff #1] you left [Child A] on the bus".
3. Staff #2, Staff #3 and Staff #4 stated Witness #1, an individual walking through the parking lot, tapped on the window and said ?There is a child on the bus?. Staff #2, Staff #3 and Staff #4 stated [Child A] was on the bus.
4. Staff #3 and Staff #4 stated it was approximately 8:00 AM when Child A was discovered on the bus. Staff #3 and Staff #4 stated Child A was probably on the bus approximately 15 to 20 minutes.
5. The transportation attendance log for the morning of Friday, June 28, 2019 indicated there were five children being transported on the bus. Child A was documented as getting on the bus at 6:28 AM.
6. A member of management stated the driver is supposed to walk to the back of the bus and push the button that turns off the alarm to ensure everyone is off the bus. Staff #1 stated an older child pushed the button to stop the alarm on that day.
