An unannounced Code inspection was conducted today, 9/17/19.
A review of the code requirements was conducted and a review of staff and children's records.
There are two classrooms with 14 children ranging in age from 3-4 years old with two staff.
The children were learning about the letter A and then went outside for a snack on the picnic table.

Evidence:
1. Staff # 1 & 2 do not have a sworn statement or affirmation form on file.Plan of Correction: The director will have all staff complete a sworn statement or affirmation form.
Standard #: 63.2(17)-1716-A Description: Based on review of information posted the center failed to have all the required information posted.
Evidence:
1. The center did not have posted the fact that it is exempt from licensure.Plan of Correction: The director will have this posted on the parents bulletin board.
Standard #: 63.2(17)-1716-A-6 Description: Based on review of the written information for parents the center failed to have the following information in writing.
Evidence:
1. The center did not have in writing the following information for parents: enrollment capacity, health requirements for staff, public liability insurance.Plan of Correction: The director will have this information in writing for the parents.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of staff records the center failed to have all the required background checks.
Evidence:
1. Staff # 1 & 2 do not have a fingerprint-based criminal history check determination letter on file.
Staff stated they began work on 9/4/19.Plan of Correction: The director will contact OBI and have all staff submit to fingerprint background checks through Fieldprint.
