An unannounced monitoring inspection was conducted on September 25, 2019 from approximately 9:15 am-12:00 pm. Upon arrival to the facility, the children were having circle time before transitioning into playing in centers. The children were also observed having snack and listening to a story.Two classrooms and one bathroom were inspected. The center does not offer transportation. Two staff records were reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on record review, in two of two staff records reviewed, the center did not ensure that staff have a sworn statement or affirmation completed before employment.
Evidence: The Director and Church Secretary confirmed that there were not completed sworn statements for Staff #1 and Staff #2 both hired 9/1/19.Plan of Correction: We will fill out the forms and send by Friday.
Standard #: 63.2(17)-1716-B-7 Description: Based on staff interview, the center did not establish and implement procedures regarding reports of serious injury or death of children attending the childcare center.
Evidence: The Director confirmed that the center had not developed the following procedures:
1.Ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner.
2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually.
3. Reports of deaths shall be submitted no later than one business day after the death occurred.Plan of Correction: Director will create the procedures.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, in two of two staff records reviewed, the center did not ensure that staff submit to fingerprinting and obtain results prior to employment.
Evidence: The Director and Church Secretary confirmed that fingerprint results had not been obtained Staff #1 and Staff #2 both hired 9/1/19.Plan of Correction: We will get the code and get the fingerprints.
