63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted Monday, September 16, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:00 AM and concluded at approximately 2:00 PM. A total of 51 children were present during the inspection and the following staff to child ratios were observed:
Infants I: 3 children and 1 staff
Infants II: 5 children and 2 staff
Toddlers: 6 children and 2 staff
Twos: 9 children and 2 staff
Threes: 10 children and 1 staff
Pre-K I: 10 children and 1 staff
Pre-K II: 8 children and 1 staff
School Age children were not present during the inspection.
Children were observed playing with toys, playing outside, creating collages from magazines, having books read to them, playing with Play-Doh, building with blocks, washing hands and taking bathroom breaks, eating lunch and resting at nap time. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Violations:
Standard #: 63.2(17)-1716-B-4
Description: Based on the review of five children's records, the center did not ensure that all children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.

Evidence:
The record for Child #3 (first day of attendance 08/12/19) did not have documentation of immunizations. A member of management confirmed this information while the inspector was on site. The parent handbook states ?immunization records are due prior to the first day of attendance?.

Plan of Correction: We have previously requested this information from the parent. An email was sent while the licensing inspector was on site requesting the information again.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five staff records and interview, the vendor did not obtain satisfactory results of the finger-print based national criminal background check for one staff prior to beginning employment.
Evidence:
The record for Staff #5 (hire date 09/03/2019) contained satisfactory results of the finger-print based national criminal background check dated 09/13/2019.

Plan of Correction: We will ensure before any additional staff begin working we receive the results of the criminal background check first.
