Comments:
An unannounced monitoring inspection was conducted Tuesday, September 24, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 9:30 AM and concluded at approximately 12:30 PM. A total of 36 children were present during the inspection and the following staff to child ratios were observed:
Threes I: 10 children and 1 staff
Threes II: 10 children and 1 staff
Fours I: 7 children and 1 staff
Fours II: 9 children and 1 staff
Children were observed playing with toys, building with legos, painting a picture of a fire truck, learning about the letter ?Bb?, playing a memory game, reviewing the weather and calendar, participating in a visit from a fire fighter to learn about safety, washing hands and taking bathroom breaks and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of five staff records, the center did not ensure that all staff and volunteers had a sworn statement of affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
The records for Staff #1 (hire date 08/1997), Staff #2 (hire date 08/1986), Staff #3 (hire date 08/2016), Staff #4 (hire date 08/2003) and Staff #5 (hire date 08/2016) did not contain a completed sworn statement and a search of the central registry. A member of management confirmed this information during the inspection.

Plan of Correction: We will have the staff complete the sworn statements and central registry within 10 days and submit proof of the request to the licensing inspector.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation and interview, the center did not describe in written statement to parents or guardians and make available to general public aspects regarding the center's operations concerning physical facilities and public liability insurance.

Evidence:
Documentation that is given to parents and guardians was reviewed. The information reviewed did not contain information describing physical facilities and public liability insurance.

Plan of Correction: We will complete the sample form that was given to us during the inspection and add it to the parent handbook as an addendum.

Standard #: 63.2(17)-1716-B-3
Description: Based on a review of records and interview, the center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings to exclude children as necessary.
Evidence:
Five staff records were reviewed and did not contain documentation of daily simple health screening. A member of management stated the staff had not officially been trained to conduct daily simple health screenings.

Plan of Correction: We will have all of the staff complete the online daily health screening training during a staff meeting.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five records and interview, the center did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018 for five staff hired prior to 1/22/2018.
Evidence:
The records for Staff #1 (hire date 08/1997), Staff #2 (hire date 08/1986), Staff #3 (hire date 08/2016), Staff #4 (hire date 08/2003) and Staff #5 (hire date 08/2016) did not contain satisfactory results of the finger-print based national criminal background check. A member of management stated the staff have not scheduled appointments to have the finger-print based national criminal background checks completed as of 09/24/2019.

Plan of Correction: We will contact the Office of Background Investigations and obtain the information and begin the process of obtaining the national background checks. This information will be submitted to the licensing inspector within 10 days.
