An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted on September 28, 2018 from 8:45 AM and 11:10 AM with the director. Twenty-three children, ages 3 years through 5 years old, were under the care and supervision of 3 staff. Children were observed eating morning snack brought from home and doing instructional work at their desks. Center documents, five children's records, and 5 staff records were all reviewed.

Standard #: 22VAC40-191-40-D-2 Description: Based on a review of five staff records, the center failed to obtain central registry reports for staff. Evidence: Staff B (hire date 8/31/15) , staff C (hire date 8/31/09), and staff D (hire date 8/27/18), and staff E (hire date 2/11/14) did not have documentation of central registry report in their records. Plan of Correction: The director stated that human resource staff sent out the wrong forms for the central registry background checks, The center was in the process of completing new request forms for central registry background check.
Standard #: 63.2(17)-1716-A-3-d Description: Based on observation, the center failed to ensure there was one staff member for every ten children from ages 36 months to five years old. Evidence: There were twelve children in the care of one staff in the K3 (three year old) classroom. Plan of Correction: The center is in the process of hiring new staff.
Standard #: 63.2(17)-1716-A-4 Description: Based on a review of five staff records, the center failed to obtain documentation that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: Staff A, C, and E did not have documentation of annual health certifications in their files. Staff B's most recent health certification was dated 9/20/17. Plan of Correction: The center is in the process of collecting completed health certifications from their current staff.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on a review of five staff records, the center failed to obtain national fingerprint background checks for staff. Evidence: Staff A (hire date 5/18/15), staff B (hire date 8/31/15), staff C (hire date 8/31/09), and staff D (hire date 8/27/18), and staff E (hire date 2/11/14) did not have documentation of a fingerprint background check in their records. Plan of Correction: The center contacted VDSS for the provider number needed to schedule fingerprinting. VDSS staff informed the director that the provider number would emailed to the center. The center never received the provider number and the fingerprinting was not completed. The center will prioritize completing the fingerprint background checks.
