An annual subsidy health and safety inspection (SHSI) was conducted on September 26, 2019 from 8:45 AM to 11:10 1M Five staff records and five children's records were reviewed. Based on observation and documentation review, four violations of subsidy requirements were issued.

Standard #: 22VAC40-665-520-B Description: Based on a review of five children's records, the vendor did not ensure that all of the required information was present in the children's records. Evidence: Child A, C, D, and E were missing the address and phone number for the parent?s place of employment. Child A, B, C, D and E were missing the address for one emergency contact. Plan of Correction: The center will work on updating their paperwork to obtain the missing information.
Standard #: 22VAC40-665-530-3 Description: Based on a review of five staff records, not all staff had documentation of tuberculosis (TB) tests or screening. Evidence: Records for Staff A, B, D, and E did not include documentation of TB tests or screenings. Plan of Correction: The center will ensure that staff will complete a tb screening and the report will be placed in their records.
Standard #: 22VAC40-665-530-4 Description: Based on a review of documentation, not all of the staff held current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: Staff B's most recent CPR certification expired 9/19/2017. Staff D's most recent CPR certification expired 5/22/2016. Plan of Correction: The two staff that do not have current CPR certification have already been scheduled for CPR training.
Standard #: 22VAC40-665-580-B Description: Based on a review of five staff records, the vendor did not ensure that all staff completed Virginia Preservice Training for Child Care Staff within the first 90 days of employment or subsidy vendor approval. Evidence: Staff C (hire date 8/31/09) and D (hire date 8/27/18) did not have documentation of preservice training in their records. Plan of Correction: The vendor will request the documentation for the preservice training from their staff members.
