Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on September 25, 2019 from 10:00 a.m to 11:50 a.m. At the time of arrival the following staff to child ratios were observed:
6 children, 3 years old, with 1 staff
4 children, 3 years old, with 1 staff
3 children, two years old, with 2 staff
8 children, five years old, with 1 staff
5 children, four years old, with 1 staff
Classroom observations were made and children were observed engaged in learning centers, having circle time discussions, eating an afternoon snack which children bring from home and playing on the outdoor play area. Nine staff records were reviewed, handbooks and posted information were reviewed and the center program director was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space, were observed.
Staff report that the program does not transport children in a vehicle while in care.

Description: Based on a review of records and interview, the religious exempt program did not obtain results of a Search of the Central Registry for all employed staff.
Evidence:
Nine staff records were reviewed.Five of the nine staff have been employed at the center more than 30 days.
None of the five staff who were employed more than 30 days had the results of a Search of the Central Registry in their record or available for review. The Director identified staff #1, #4 and #6 as having been hired September of 2018 and staff #5 and #9 as having been hired 2 years ago. The remaining 3 staff have been employed less than 30 days.

Plan of Correction: The program director has already sent requests for a Search of the Central Registry for all nine center staff and had supporting documentation. The program director confirmed that she does not know whether central registry searches were previously requested before her employment as director and was unable to locate such documentation. Upon receipt of the results, they will be reviewed and filed in each staff record and a copy will be forwarded to the inspector.
