The licensing inspector for DEMI/Good Shepherd School and Day Care conducted an unannounced non-mandated subsidy health and safety inspection in response to an injury that occurred in the center on 9/10/19 and was reported by the center on 9/13/19. A violation of the religious exempt requirements was found during the investigation and is documented on the violation notice. Standard #: 22VAC40-665-550-2 Description: Based on a review of an injury report and interview, the center failed to inform the department's representative as soon as practical, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
1. A serious injury report was received by the department on 9/13/19. The date of the injury was documented on the injury report as 9/9/19.
2. The director stated the date on the report was an error. The date of the injury was 9/10/19. The director stated she was unaware they were to report serious injuries within two days.Plan of Correction: In the future the center will inform the department's representative as soon as practical, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
