An unannounced monitoring inspection was conducted Wednesday, October 2, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 9:00 AM and concluded at approximately 12:10 PM. A total of 31 children were present during the inspection and the following staff to child ratios were observed:
Twos (Caterpillars): 2 children and 2 staff
Twos and Threes (Butterflies): 10 children and 2 staff
Threes and Fours (Bumblebees): 5 children and 2 staff
Fours and Fives (Ladybugs): 14 children and 3 staff
Children were observed playing with toys, participating in circle time, reviewing the calendar, playing outside, learning about bugs, having their diapers changed, washing hands and taking bathroom breaks and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Standard #: 22VAC40-191-40-D-2 Description: Based on review of five staff records, the center did not ensure that all staff and volunteers had a sworn statement or affirmation prior to the beginning of employment and a search of the central registry within the first 30 days of employment or volunteering.
Evidence:
The records for Staff #1 (hire date 09/14), Staff #2 (hire date 09/17), Staff #3 (hire date 09/18), Staff #4 (hire date 9/3/19) and Staff #5 (hire date 9/3/19) did not contain central registry results and a sworn statement. A member of management stated she was not aware this was a requirement for staff members.Plan of Correction: We have the national based criminal record check for all staff members. I was not aware of this requirement. We have a staff meeting tomorrow and I will go over the paperwork with the staff and submit the requests to the licensing inspector within 10 days.
