Comments:
An unannounced Subsidy Health & Safety inspection was conducted Thursday, October 3, 2019 to determine the center's compliance with Subsidy Health and Safety vendor requirements. The inspection was initiated at approximately 9:45 AM and concluded at approximately 2:30 PM. A total of 24 children were present during the inspection and the following staff to child ratios were observed:
Infants: 7 children and 2 staff
Toddlers: 7 children and 2 staff
Threes and Fours: 10 children and 1 staff
School Age children were not present during the inspection.
Children were observed playing with toys, sleeping in cribs, reviewing numbers, washing hands and taking bathroom breaks, eating lunch and resting at nap time. Five children?s records and five staff records were reviewed. Violations were cited during the inspection. See the violation notice for additional information. Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on review of five children's records, the vendor did not ensure a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction was obtained.

Evidence:
The records for Child #3 and Child #4 indicated each child was allergic to a food. Staff #4 stated the parent's have not submitted an allergy care plan.

Plan of Correction: I will have parent get written action plan or remove it from file by the parent.

Standard #: 22VAC40-665-650-F
Description: Based on observation and interview, the vendor did not ensure staff were not counted in required staff-to-child ratios unless they are directly supervising children.
Evidence:
At approximately 10:00 AM there were 17 children (23 months to 4 years of age) being supervised by Staff #1 and Staff #2 in the Twos classroom. Staff #3 was observed sweeping the School Age classroom and entrance at approximately 10:00 AM. At approximately 10:05 AM Staff #3 returned to the Twos classroom. When Staff #4 arrived at the center at approximately 10:15 AM, she stated the children were combined until she and another staff person arrived and the children were being supervised by Staff #1, Staff #2 and Staff #3.

Plan of Correction: I will remind staff to remain in class until other staff can relieve them.

Standard #: 22VAC40-665-740-A-2
Description: Based on observation, the vendor did not ensure children?s hands were washed after diapering
Evidence:
Staff #1 was observed changing a child?s diaper in the Two?s classroom. After finishing the diaper change, Staff #1 removed the child from the diaper changing table and the child went back to playing with toys in the classroom.

Plan of Correction: Have staff review safe hand washing practice for children after diapering.

Standard #: 22VAC40-665-740-B-8
Description: Based on observation and interview, the vendor did not ensure the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
Staff #1 was observed changing a child?s diaper on the changing table in the Two?s classroom. After the diaper change, the staff washed her hands and began caring for the children in the classroom. Staff #1 was asked the process for cleaning the diaper changing table. Staff #1 stated it is cleaned with Clorox but the spray bottle is broken.

Plan of Correction: Review training and make sure they have both materials available. Retrain worker on proper diaper changing procedures.
