63.2(17) License & Registration Procedures

Comments:
An unannounced compliance monitoring inspection was conducted from 9:30 a.m.-12:30 p.m. 12 children, ages 3 years through 4 years old, were under the care and supervision of 3 staff. Children were observed completing worksheets for community helpers' week, during transition time and reading quietly. Center documents and four staff records were reviewed.

Standard #: 22VAC40-191-40-D-2
Description: Based on review of documentation, it was determined the center did not have documentation of sworn statement or affirmation before employment. Evidence: Staff #2, hired in 2017, did not have documentation of a sworn statement in the record.

Plan of Correction: Staff #2 completed the sworn statement during the inspection.

Standard #: 63.2(17)-1716-A-6
Description: Based on review of documentation and interview with the director, the center did not provide a written statement to the parents or guardians of the center and the general public regarding public liability insurance. Evidence: There was no written statement regarding public liability insurance.

Plan of Correction: This information will be added to the program's website.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of documentation, it was determined that the center did not have documentation of repeat background checks every five years as required. Evidence: Staff #3, hired in 2014, most recent sworn statement was dated 3/13/2014 and most recent central registry background check was dated 4/1/2014.

Plan of Correction: Sworn statement will be completed today. Staff #3 has already been given the central registry background check and it will be completed as soon as possible.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of documentation, it was determined that the center did not obtain fingerprint documentation within the required implementation of September 30, 2018. Evidence: Staff #2 fingerprint background check was dated October 25, 2018. Staff #3 fingerprint background check was dated October 2, 2018.

Plan of Correction: Information regarding the implementation of fingerprints was sent to an old email address. The email has been updated and fingerprint procedures implemented for our program. Any future employees will have documentation of fingerprint background check before employment.
