An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 12:00 p.m. with the director. There were 34 children in care ranging in age from 3 to 5 years of age under the care and supervision of 5 staff. Children were observed during morning snack, restroom break and story time. Center documents and six staff records were reviewed.

Standard #: 63.2(17)-1720.1-A Description: Based on review of documentation, it was determined the center did not ensure staff had documentation of repeat background checks every 5 years as required. Evidence: Staff #6, start date 2015, most recent sworn statement was dated 7/10/2013. Plan of Correction: Staff #6 completed a sworn statement during the inspection.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of documentation and interview with staff, it was determined that the center did not obtain fingerprint background check results by the required implementation date of 9/30/2018. Evidence: Staff #3, start date September 2018, fingerprint background check was dated 10/17/2018. Plan of Correction: In the future, all background check timeframes will be followed.
