An unannounced monitoring inspection was conducted beginning at 10:30 a.m. and ending at 12:00 p.m. with the director. There were 22 children in care ranging in age from 3 to 5 years of age under the care and supervision of 3 staff. Children were observed during morning snack, restroom break, a craft project and free play. Center documents and six staff records were reviewed.

Standard #: 63.2(17)-1716-B-3 Description: Based on review of documentation and interview with staff, daily simple health screenings and exclusion of sick children were not conducted by a person trained to perform such screenings. Evidence: No staff had documentation of Daily Health Observation (DHO) training. Plan of Correction: Daily health observation training will be scheduled as soon as possible.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of documentation and interview with staff, it was determined that the center did not request out of state central registry background checks within 30 days of employment for staff that resided outside of Virginia in the past 5 years. Evidence: Staff #1, first day of employment 8/19/2019, indicated out of state residence in Alaska in the past 5 years and there was no documentation of an Alaska central registry background check request. Plan of Correction: An Alaska central registry background check will be conducted as soon as possible.
